When you buy or sell your home, the name and address on the tax bill is not automatically changed. The new owner must submit a Name/Address Change Application to the Cook County Treasurer's Office with acceptable identification. If this application is submitted by mail, your signature must be notarized.

Select from one of these 3 options to complete the name-change process:

1. When paying your property tax bill, fill out the back portion of the tax bill stub with the new name and/or billing address. Have it notarized and mail it with your payment.
   
    
2. Complete a Mail-In Application for Change of Name or Address. Forms are available at our office and can be downloaded from this Web site under the "Forms" section. Mail-in forms must be notarized and addressed to our main office.
   
    
3. If you submit the form in person without notarization, you must bring your 14-digit Property Index Number (PIN) and TWO of the following forms of identification:

• Driver's license
• State identification card
• Social security card
• Passport
• Birth certificate
• Military license or identification card
• Current certified copy of trust agreement with notarized letter
• Employee photo identification card with a check stub
• Copy of recorded deed to property or articles of agreement for deed
• Notarized power of attorney reciting authority to change name or address

NOTE: Our office cannot change the mailing address information to the address of a mortgage company, bank or other taxpaying agent who pays on behalf of the taxpayer. When a property owner pays through escrow, it is important that the owner receive the bill for payment monitoring purposes and so that he or she receives all important tax-reducing exemption, assessment and delinquency notices.
